Use this page to schedule your consultation with our Surgeons. Click here then Select Consultation.  You can then select your surgeon.  Find the day and time of your choice and schedule your appointment.    We will confirm your appointment prior to your visit. 

 Please let us know if you are unable to keep the scheduled appointment.  If you schedule your appointment within 24 hours of the appointment time, call our office to confirm the time.

Now that you have scheduled your appointment, it will save time to download two forms.  The forms are a new patient encounter form and a medical history form.  They are needed when you arrive in our facility.  To have them already filled out will save time and give you more time with your surgeon.

You will need Acrobat Reader version 7 for XP, a free software program to view the forms.    When your acrobat reader program opens, print the forms on your printer.  Fill out the forms and bring them with you to your scheduled consultation.
